Notification of Data Security Incident
October 25, 2024 - On or around August 27, 2024, Oregon Reproductive Medicine LLC dba ORM (“ORM”), experienced a network disruption that impacted certain systems and immediately began investigating with the assistance of third-party computer forensic specialists through legal counsel. The investigation determined that certain data was accessed by an unauthorized party from August 26, 2024 to August 27, 2024. This data does not include ORM's Electronic Medical Records (EMR), email, CRM, or financial or insurance information, as such systems were not accessed. We are currently reviewing and analyzing the types of information potentially impacted by this incident; however, ORM has confirmed that a limited amount of protected health information may have been impacted in connection with this incident.
Although the review is ongoing, the types of information impacted by this incident might have included lab data and patient names. ORM has established a hotline to answer questions about the incident and to address related concerns. The number for the hotline is 855-285-7634. In response to this incident, ORM has implemented additional security measures intended to prevent similar incidents from happening in the future and we will continue to monitor and enhance our security measures. Although ORM has no evidence of actual misuse of information as a result of this incident, we are notifying you out of an abundance of caution and it is always prudent to review health care statements for accuracy and report any services or charges that were not incurred to the provider or insurance carrier. As a best practice, individuals are encouraged to remain vigilant against incidents of identity theft by reviewing account statements for suspicious activity and to detect errors. We recommend you review the below “Additional Resources” section below which describes additional steps you can take to help protect yourself, including recommendations by the Federal Trade Commission regarding identity theft protection and details on how to place a fraud alert or a security freeze on your credit file.
